Job summary

NHFT have an exciting opportunity to recruit a Band 4 Community Support Worker to work into the North Northants Council Rough Sleeping Team.

The Council's Rough Sleeping Team work across North Northants, offering support to Rough Sleepers. The postholder will be key member of this integrated team delivering mental health support out in the community.

The postholder will be supervised by the Mental Health Practitioner in the team and work in partnership with other members of this integrated team.

Being part of an integrated team, the role will involve proactively engaging with service users during outreach sessions and drop in services as a means of fostering a trusting therapeutic relationship

Main duties of the job

The development of this position will seek to ensure that we are better meeting the mental health needs of some of the most hard to reach and vulnerable members of our community.

It has been evidenced that more than half of those sleeping on the streets have difficulties with their mental health. Vulnerable rough sleepers can often face a 'revolving door' where they receive treatment and support, only to end up back on the streets.

Mental health can be the reason that some people become homeless, and being on the streets can further negatively impact on individuals' mental health meaning that self-management of their mental health symptoms can be further challenged.

The hope is that people who are street homeless or have homelessness difficulties will benefit from an assertive outreach model of support;

the postholder will be a key member of the team in providing this support.

Job description

Job responsibilities

The role of Community Support Worker is to deliver mental health interventions and support to service users who are experiencing rough sleeping as part of a co-produced care plan overseen by the Mental Health Practitioner.

All interventions administered and support offered by the post holder will be under direct and indirect supervision by the Mental Health Practitioner and other members of the integrated Rough Sleeping Team.

The Community Support Worker will contribute to the ongoing assessment, planning, implementation, and evaluation of care provided to the service users they are working with.

The role will involve delivering in and outreach support.

Person Specification

Knowledge / Experience

Essential

  • NVQ L3 or equivalent experience
  • Working in a community mental health setting

Desirable

  • Working with people who are experiencing a crisis
  • Working with people who have complex needs
